Follow-up of patients with common non-communicable diseases

Follow up of patient's especially chronic non communicable diseases sufferers is very important for early detection and treatment of complications. During your community visit, you need to see & communicate with patients who have follow up and on medication. Appoint patients who needs frequent follow up to your health post for further check up. Asking the patient whether he/she takes the prescribed medications as ordered or not. Seeing the symptoms developed either due to the drug side effects or the disease complications and immediate action should be taken.
