Report writing

Report writing is among several important activities the health extension worker should do based on the need of the existing structure in the health sector including other stakeholders. Therefore, the health extension worker should use the available formats and guidelines to prepare comprehensive reports of the activities performed in adolescents and youth reproductive health package. By Compiling daily performed activities you should prepare and submit monthly, quarterly and annual reports to the woreda health office. The report compilation can be done using the existing national or regional standard formats.

The reports should mainly contain:

• Introduction.
• The achievements (accomplishments regarding the set objectives).
• Outcomes (the observed results and changes).
• Benefits (advantage it provides to the service users).
• Problems encountered during the implementation of the package and
• Summary.
